Here are the requirements to be a surrogate:
-- Must be aged 21-43.
-- Must have a BMI (Body Mass Index) of less than 25 (25-30 will be reviewed).
-- Must be medically appropriate.
-- Before an appointment is made for a gestational carrier to meet a physician, our fertility clinic must review the gestational carrier's obstetric records and have received a letter from the gestational carrier's obstetrician stating that she is cleared for a surrogate pregnancy.
-- Will not be approved if she has had more than two C-sections unless our fertility clinic receives medical clearance from an OB MD stating surrogate has been counseled of risks to fetus and her own health with more than two C-sections. Intended parents must be counseled by the OB MD as well and need written notice for my fertility clinic.
-- Must have had at least one successful live birth and is currently parenting her child or children.
-- No previous ectopic pregnancies (a pregnancy that is not in the uterus)
-- No more than 5 prior deliveries. 
-- Must have had a normal PAP within the last year. 
-- Is required to have a psychological screening, which must include the gestational carrier's partner, if applicable. 
-- No essure clips (a type of birth control).
